Case Management – Social Worker III
As a Social Worker III Case Manager, you’ll partner with patients, families, and the care team to support safe, timely transitions across the continuum of care. You will assess psychosocial and discharge needs early, coordinate services and community resources, and advocate for patient-centered plans that improve quality outcomes and responsible use of resources. This role uses a multidisciplinary approach to arrange post-acute services, monitor progress throughout the hospital stay, and contribute to performance improvement initiatives across Berkshire Health Systems.
Required: LCSW (Licensed Clinical Social Worker). Preferred: LICSW (MA Licensed Independent Clinical Social Worker).
Health Education – Oncology Social Worker III
The Oncology Social Worker III provides psychosocial assessment, counseling, and resource navigation for patients and families throughout the cancer care continuum. Working closely with the multidisciplinary oncology team, you’ll incorporate psychosocial needs into care planning, complete distress screenings, provide crisis intervention, and connect patients to supportive services. You will also assist with complex situations that may include financial concerns, legal issues, and required reporting, and you’ll document services in alignment with organizational policies and quality requirements.
Required: LCSW. Preferred: LICSW and Oncology Social Work Certification.
Crisis Screening – Psychiatric Emergency Department Screener
The Psychiatric ED Screener provides rapid triage, assessment, and disposition planning for individuals presenting to the Emergency Department in mental health crisis, voluntarily or under Section 12A. In partnership with ED clinicians, psychiatry providers, community agencies, and safety personnel, you’ll complete psychosocial and risk assessments, support crisis de-escalation, and coordinate next-step levels of care, including referral and placement. This role requires strong clinical judgment, clear documentation, and working knowledge of DSM-5 diagnostic criteria.
Required: LSW, LICSW, or LMHC (or equivalent independently licensed mental health credential, as applicable).
Youth Behavioral Health Clinician – Behavioral Health
The Youth Behavioral Health Clinician provides assessment and therapeutic services within our Youth Intensive Outpatient Program (IOP) for adolescents ages 12–17. In this structured, time-limited outpatient setting, you’ll deliver evidence-informed, milieu-based care as part of a multidisciplinary team that includes child psychiatrists and licensed clinicians. You will help stabilize symptoms, strengthen coping skills, engage families as appropriate, and coordinate with outpatient providers to support continuity of care and reduce avoidable ED visits or inpatient admissions.
Required: LICSW (MA) or LMHC (MA).
Primary Care Integration Clinician – Behavioral Health
The Behavioral Health Integration (BHI) Clinician is an independently licensed behavioral health professional embedded within primary care. You’ll collaborate with primary care teams to identify and address behavioral health needs that affect overall health, including brief assessment, screening, and short-term, evidence-based interventions. Typically, you’ll support patients over 4–6 visits, coordinate referrals for ongoing specialty care when needed, and provide consultation to primary care clinicians. You will also partner closely with psychiatric providers to facilitate diagnostic evaluation, medication recommendations, and communication across the care team.
Required: LICSW (MA) or LMHC (MA).
Clinical Therapist – Behavioral Health Acute Setting
The Clinical Therapist is a key member of an interdisciplinary inpatient behavioral health team, providing comprehensive psychosocial assessment and therapeutic interventions in a structured, safety-focused setting. You’ll facilitate individual, group, and family therapy, contribute to treatment planning, and continuously reassess progress to ensure care plans meet patient needs. This role requires strong communication skills, sound risk assessment, and timely, high-quality documentation in the medical record.
Preferred: LSW.
Care Coordinator – Behavioral Health
The Behavioral Health Care Coordinator provides specialty care management for high-risk patients enrolled in the Health Collaborative of the Berkshires (ACO-Medicaid). You’ll partner with patients, families, primary care providers, and care teams to assess needs and strengths, develop proactive care plans, and coordinate behavioral health and substance use supports. The role includes brief intervention, ongoing monitoring of plan effectiveness, and coordination across hospital, community, and practice-based services. Patient outreach may be completed by phone, in clinic, or in the home based on individual needs.
Required: LCSW, LICSW, or LMHC.
