1. Diagnose and treat acute illnesses and chronic disease exacerbations. 2. Coordinate all aspects of a patient’s care during hospitalization 3. Evaluate patients in the Emergency Department and decide on hospital admission. 4. Manage discharge planning, including medication reconciliation, follow-up arrangements, and patient. education. 5. Serve as the central point of contact among specialists, nurses, therapists, and primary care providers. 6. Communicate clinical updates and plans with the patient’s family and outpatient doctors. 7. Perform common bedside procedures such as lumbar punctures, thoracentesis, paracentesis, and central line placements. 8. Facilitate transitions to rehab, skilled nursing facilities, or home care services with proper handoff. 9. Respond to acute changes in patient conditions (e.g., rapid response calls, codes). 10. Stabilize patients until ICU transfer or specialist input.
