Because anesthesia and surgery affect every system in the body, your anesthesia provider will conduct a preoperative evaluation. Sometimes this interview is initiated by a registered nurse in person; in other cases, you will be interviewed over the telephone. During this interview they will review your medical history and any other pertinent information needed to safely administer your anesthetic. He/she will also inform you about what to expect during your surgery. On the day of your surgery, the anesthesia team will meet with you before your surgery to review your entire medical history obtained during the interview as well as results of any medical tests conducted during your preoperative evaluation. It is important to discuss any changes in your health that may have occurred in the days preceding your surgery, such as fevers, chills, upper respiratory infections, vomiting, diarrhea, or other ailments.
Online Medical HistoryTo start the pre-surgical assessment process for endoscopy procedures or surgery at the Crane Center for Day Surgery, Berkshire Medical Center requests that you fill out your medical history online with One Medical Passport. We recommend that you enter your medical history online as soon as your surgery has been scheduled. Once you do this, our Pre-Surgical Assessment nurse will be able to access the information you entered online. This information will assist the nurse in organizing and documenting your complete medical history to prepare for your surgery.
Be sure to have the following information available before starting One Medical Passport: • Your health insurance information.• The names, addresses and phone numbers of your physicians.• A list of all medications you are taking, their dosage and frequency.• A list of surgical procedures you have ever had and their approximate dates.• The name of the surgeon who will be performing your upcoming procedure at Berkshire Medical Center.
