What is Berkshire Fallon Health Collaborative (BFHC)?
BFHC is your new health plan as of March 1, 2018 if you’re on MassHealth and have a Primary Care Provider (PCP) at Berkshire Health Systems.
Overview
BFHC is a MassHealth Accountable Care Organization (ACO) Partnership Plan. It’s made up of Berkshire Health Systems (BHS), several Berkshire County community physician practices, Community Health Programs (CHP) and Fallon Health. Fallon Health is a leading health care services company that has been caring for MassHealth members for over 40 years. Their mission is to make their communities healthy. As a member, all of your health care is managed by your PCP. Your PCP will work with BFHC’s group of doctors, medical and community health centers to give you great health care.
What’s an Accountable Care Organization (ACO)? An ACO is a health plan that’s accountable to give quality care. And help you meet your health goals. ACOs have groups of PCPs and other providers. They work together to meet your overall health care needs. In an ACO, your PCP and their team is responsible for working with you and your other providers. They’ll help coordinate your care and connect you with available services and supports. This coordination will help you get the right care at the right time.
How do I know if my PCP is part of BFHC? Most Berkshire county PCPs are part of BFHC. Call BFHC or visit our website http://www.fchp.org/Berkshires.aspx.
Do I need to do anything to enroll in BFHC? If you have a PCP that is participating in BFHC, then MassHealth will enroll you in BFHC for 3/1/2018. You shouldn’t need to do anything else.
How will I know for sure if I’m in BFHC starting 3/1/2018? You’ll get a letter from MassHealth. It will come in November or December of this year. If the letter says your PCP has joined BFHC, you’re enrolled in BFHC. You don’t have to do anything. The letter will come in an envelope with a big green stripe. Be on the lookout for it! Will I pay more for BFHC?
No. There’s no cost to join BFHC.
Will my MassHealth benefits change with BFHC?
You’ll have the same benefits you’ve always had with MassHealth, including behavioral health services, plus you’ll get:
BFHC can also help you get counseling services. They can teach you about children’s services. And they can show you how to apply for food stamps and much more.
Where do I go for care as a member of BFHC?
You’ll keep seeing your doctors in their offices, just as you do today.
Does anything change for me before 3/1/2018?
No. Your MassHealth plan will stay the same through 2/28/2018. BFHC will become your MassHealth plan starting on 3/1/2018.
My specialists are part of BHS. My PCP isn’t. Can I keep seeing my PCP when I have BFHC?
Maybe. It depends if your PCP’s practice lets patients see specialists who aren’t in their network of doctors. To be sure you can stay with your BHS specialists, you’ll need to register with a BFHS PCP. You must also get enrolled in BFHC.
