One of the biggest challenges for anyone living in a community, is getting connected to the clinical and community resources they need when they need them! We may know about some of these resources because we have had interactions with a given organization. However, we simply cannot know it all and most of the time we can’t absorb the information until we need it. Jamal Miles, our Community Health Worker (CHW) at Fairview, is available to help get people connected to the resources they need when they need them. He provides a service every Wednesday afternoon from 2-4 pm in the Administrative Conference Room at Fairview. The program is called Community Health Connections and is open to anyone including Fairview employees, patients, and the general public. The World Health Organization (2007) reported evidence from the literature which revealed that the use of CHW’s in a community leads to improved health outcomes .
