The Admitting Department is located on the Ground Floor by the Main Entrance providing two private interview rooms to ensure a private registration process.
Our trained staff is available to help you from 7:30a.m. until 8:00p.m. Monday through Friday and 8:00 a.m. to 7:00 p.m. Saturday and Sunday.
During the registration process you will be asked questions to assure your health information is correct. This information is necessary and we appreciate your cooperation as we work together as a team to address your healthcare needs. Patients are asked to bring the most up-to-date insurance information when they register.
It is the policy of Fairview Hospital to admit all patients regardless of race, color, national origin, handicap or ability to pay.
Consent Forms and Interpreter Services
Our staff will ask you to sign permission for treatment. Parents or guardians must sign for minors. For non-English speaking patients, Fairview Hospital will provide an interpreter, whenever possible. Access to interpreters of over 400 languages is available through a language interpreter telephone service 24 hours a day. To request interpreter assistance, please ask a member of our Admitting Staff.
Fairview Hospital does not assume responsibility for loss of or damage to valuables, clothing or other personal items. We strongly advise you to leave your valuables at home. If this is impossible, please ask a nurse to deposit your valuables in the hospital safe. If you have eyeglasses or contact lenses, please keep them in a container.
Maternity patients are asked to pre-register 3 to 6 weeks prior to their expected delivery date.
