Your primary care team may consist of a physician, nurse practitioner, physician assistant, community health worker, nurse care manager, and behavioral health clinicians, who are all skilled in identifying and treating common conditions and ailments.

From performing routine check-ups and screenings, to diagnosing common conditions and coordinating specialty care, BHS’s expert primary care providers are here to help patients get and stay healthy.
