Case Management and Care Transitions at BMC
Berkshire Medical Center is genuinely committed to assuring patients’ safe transitional care needs throughout a seamless “Continuum of Care.” Case Managers at BMC are Registered Nurses and Licensed Social Workers who provide comprehensive care coordination and discharge planning for patients discharging from BMC.
Berkshire Medical Center provides an “interdisciplinary team” approach comprising physicians, nurses, Case Managers, therapists, dieticians and social workers to manage patient care, improving care delivery that promotes high quality and cost effective care. Collaboration and effective communication among the patient and caregivers, healthcare professionals and health insurance plans in all care settings is necessary to achieve positive outcomes and exceptional patient experiences.
Hospital Care Coordination provides services with an individualized, patient centered focus. This effort breaks down barriers and makes health care for our patients easier to understand and manage. Case Managers provide admission, on- going hospitalization and discharge assessments to identify high risk patients and their complex care needs. These personalized services evaluate patient safety, medical stability, psycho-social, financial and legal issues. Case Managers help facilitate the patient’s hospital care delivery and execute appropriate community referrals to enhance safe transitions back to the community.
Case Managers at BMC possess advanced knowledge of government and commercial insurance benefit allowances and limitations and work closely with patients, their families and healthcare providers providing guidance allowing them to reach their personal goals. Case Managers assist patients and families to understand insurance regulations determining inpatient or observation status for their hospital stays. Their patient advocacy commitment is evident during the hospital discharge appeals administration. Case Managers are the clinical liaison between the patient and their insurer who provide ongoing clinical dialogue during the hospital encounter.
