Under Massachusetts law, you have the right to appoint a healthcare agent to make medical treatment decisions for you in the event you are unable to make or communicate such decisions. The appointment of this healthcare agent must be in writing, in the form of a healthcare proxy. For the healthcare proxy to be valid at the time you sign it, you must be mentally competent and over the age of 18.
If you have questions regarding this Massachusetts law or if you would like more information and/or a healthcare proxy form, please contact the Patient Relations Office at Berkshire Medical Center at (413) 447-2466 or the Patient Advocacy Office at Fairview Hospital at (413) 854-9636.
