To paraphrase a familiar saying, “it takes a village” to help a concussion patient return to good function. Recovery begins with proper care on the day of the injury and continues in the days and weeks beyond. One of the main goals of the Berkshire Medical Center Concussion Clinic is to provide easy access to specific up-to-date information about concussions to each person in that “village” who is involved in supporting the patient during their recovery.
The drop-down menus to the left contain practical information and forms useful to Parents/Patients, Educators/School Nurses/Therapists, and Medical Providers. These forms will be updated as new information becomes available.
Any comments about content, or ways to improve the utility of these forms or this website in general are always welcome.
