Chronic Kidney Disease Clinic
We provide a comprehensive approach to the management of patients with kidney disease. The multidisciplinary team approach includes treatment by a physician, nurse practitioner, registered dietician, licensed social worker and dialysis access nurse to provide the best overall care possible. Our mission is to serve our community by providing high quality care in preventing or delaying kidney disease, cardiovascular disease, and premature death. We provide comprehensive kidney disease management with a continuum of care for the patient through all stages of chronic kidney disease.
The Chronic Kidney Disease Clinic works in conjunction with the patient's physician to provide one-on-one education about:
- Home hemodialysys
- Peritoneal dialysis
- Dialysis access
- Social issues
In addition, the Clinic works with patients on treating chronic kidney disease risk factors, such as hypertension (high blood pressure) and diabetes, with a concentration on slowing the progression of kidney disease and preventing cardiovascular disease.
We also provide treatment of electrolyte disorders, including hyponatremia (low sodium level in blood), hypernatremia (elevated sodium levels in blood), hypokalemia (low potassium levels in blood), and hyperkalemia (elevated potassium levels in blood). In addition, our Nephrologists have expertise in treating acid-based disorders.
